Request for ACT 48/45 Course Reporting
In compliance with the Pennsylvania Department of Education (PDE), Â鶹Íø will report successful completion of courses taken to satisfy ACT 48/ACT 45 regulations. The report will be sent to PDE and the professional educator (the student). Records of completion of one to six credits at Â鶹Íø Family will become part of the student's permanent transcript. To request that your course completion (1-6 credits) be reported in compliance with ACT 48/ACT 45, you must complete this form.
